Verification of Overseas COVID-19 Vaccination and Serology Testing 2023

• This service is available only to all current NUS Student, Staff and Dependents. Your Student/ Staff card or In-Principle Approval (IPA) letter, Singapore Identification card (NRIC/FIN), Foreign Identification document (e.g. Passport/ID as stated on the vaccination certificate), printed copy of COVID-19 Vaccination documentation showing the dates taken for all doses, and confirmation of appointment is required at the point of registration.
• You will be required to make payment ($25 - $40 inclusive of GST) on the day of your appointment. Cash, NETS, Credit Card, Debit Card and PayNow are acceptable modes of payment.
• Once you have scheduled a confirmed appointment, you may change the appointment 24 hours prior to the confirmed appointment date (please refer to the FAQ), thereafter rescheduling is not allowed.
• Personal data provided by the Attendees may be used by NUS for the purposes of the Event/service provided and communicating with the Attendees. By registering in the Event, you are deemed to have given your consent to the processing of your personal data by NUS and/or its service providers in connection with the Event/service provided. You acknowledge and agree to NUS’ Privacy Notice and its legal information and notices.
